Here is a description of the standard fields that make up an assumption record.

| Assumption Name | * | The name of the assumption. |
| Assumption Owner | The resource assigned as the assumption owner. | |
| Comments | Any additional comments related to assumption. | |
| Date Raised | The date on which the assumption was created. | |
| Impact | The impact of an assumption, where 1 is the lowest impact and 5 is the highest. | |
| Likelihood | The likelihood of an assumption to occur, where 1 is the least likely and 5 is the most likely. | |
| Milestone | The milestone related to assumption. | |
| Opportunity | The opportunity related to assumption. | |
| Project | The project related to assumption. | |
| Project Task | The project task related to assumption. | |
| Severity | The severity of an assumption. Calculated from impact and likelihood values. | |
| Status | The status of the assumption. Values are Active, Validated, and Invalidated. | |
| Validated | Determines when validated based on the assumption status. | |
| Validated Date | The date on which the assumption was validated. | |
| Validation Plan | A description of the plan of validation to take place against the assumption. |
